prenatal/new patients
Schedule your first appointment by phone (212-496-5437).
(Not for prenatal): Fill out the Medical Records Release Form. Send this to your child's prior pediatrician to allow them to release your child's medical records to us. We need these records before your child can be seen.
Fill out and bring the Patient Demographic & Consent Form with you to the visit.
Bring your child's insurance information and any school forms you need filled.
If you would like, review our office policies here. You will also receive these when you arrive.
current patients
Please bring your immunization records and any forms that need to be completed.
If someone other than a parent or legal guardian (babysitter, grandparent, etc.) is bringing your child to our office, you must fill out this form and send it to us prior to the appointment.
If your child has a fever, cold, or cough within 2 days of the appointment, please call us ahead of your appointment to ensure the safety of all patients.
